Access Claims and Pre-Treatment Estimates (Delta Dental)

(1) Login/create an account on the Delta Dental Members website.

How to create an account

1. From the Login page, select Create an account.

2. For User Type, select Enrollee/Adult Dependent

3. Enter your personal information, including your Enrollee ID (Usually on a previous Explanation of Benefits (EOB) or insurance card. Feel free to call the office if you can’t find it.

4. Set up your username, password, and challenge question.

5. Select Register.

(2) Click on “Legacy Portal”

(3) Click on “Claims”

(4) Click on the Claim Number

(5) Review Claim/Pre-Treatment Estimates
